Transition Services

Transition services refer to a coordinated set of activities for a student which promotes movement from school to post school activities. Some examples of post school outcomes may include: Post-secondary education, technical training, integrated employment, supported employment, adult services, independent living, and/or community participation.

Transition services are offered to support students by introducing skills across multiple environments. Students (ages 14 to 21) are taught skills in the classroom and in the environments in which these skills are naturally applied. 

Transition services assist students and their families in developing contacts with adult service providers, such as the Division of Developmental Disabilities and Division of Vocational Rehabilitation. This provides students and families a secondary transition to community supports and services upon graduation.
